Answers:    Check your roof and fix the leaks up nearby. Also, check and see if there are any pipes running along where on earth your ceiling is getting wet spots.

own you fixed the leak? if so, and the kilz isn't covering it, i would cut the box out and replace it with contemporary sheet rock.
