Many Older houses have access through a "Access Panel" in the passage or behind the shower or tub. Pipes can also be repaired using non evasive or less invasive methods such as New PEX or restoring the pipes near Curaflo's system.

Answers:    It depends on the bathroom and which pipes. On one of my bathrooms for example, most of pipes can be reached through the unfinished crypt and the rest through an access panel located in the closet. If a wall needs to be cut into later the plumber may be able to just cut out the portion of drywall where on earth the pipes are then replace it with an access panel or replace the drywall. If you hold PEX style pipes then the pipes may be able to be fished through the walls when self replaced, but from my experience this will take patience and time.
If you are chitchat about drain pipes then it is still possible that a repair will not verbs your bathroom, it would just depend on how much of the pipe needs repair, if it is contained by a concrete slab, and the skill of tour plumber.

If the bathroom is tiled, and the piping is on an inside wall, perhaps the pipes can be access by opening up the wall on the other side. Patching drywall is a snap. Depends on which pipes you want to get to and what is surrounded by front of them. If it's tile then say goodbye to the tile---at most minuscule in that section.
