Without more detail; I enjoy to ask your definition of "Torn Apart". I'll assume you mean separated from the substrate?

Certainly it would aid to know WHY? It would also help to know what the substance of the substrate is; the method of tile install; type of mortar (mastic) and the common condition of any tile that remains in place.

Do you own extra tile? Do you have tools such as a Diag. Grinder; you might use to remove ripened mortar and grout for the substrate and back of respectively tile piece?

I'm remediating 11,000 sq. ft. of tile now; installed indelicately in a commercial building crypt that floods. Tedious work; but do-able.

Answers:    use a stanley knife to scuff out the grout.remove the tile using a hammer and a bulster or a infirm chissel always work from the middle avoiding incapacitate to the other tiles.next use the tack hammer and bolster to chip up all the antediluvian adhesive subsequent spread the new tile sticky stuff on the new tile lay it and check that its not sticking out contained by front of the other tiles.make sure its plane with them.once dry after a thew hours after you can grout it.
You will have to chip out the undermined ones and replace them. After you grout be sure to apply a grout sealer. These links will give you some guidelines.
