Answers: If you are any virtuous at DIY you could do it for about lb150.
1. Buy a four foot concrete lintel from your local builders merchant, (Two if it's a cavity wall).
2. If it is a brick wall of late chisel out one course of bricks, just over four foot wide-ranging, if it is a concrete wall you can hire an electric chisel. The height of the foundation of the 'slot' should be approximately the height of the door frame which you can buy all set made from the builders merchant.
3. Cement the space between the top of the lintel and the base of the bricks, you may enjoy to pack this space with some slates etc.
4. When the cement have completely set, hack out the width of the door below the lintel, which should leave almost six inches of lintel resting on either side of the door frame, fit the frame, engineer good and buy a door of your choice, you can bring back a good second paw one from several sources.
