Answers:    If you're washing next to bleach, the bleach is slowly eating away at them. Its also a matter of shirt level. Don't use too much hot water and go unproblematic on the bleach.
I had a similar problem a couple years ago, which I finally realised was cause by my belt (I had a skate belt with the flappy catch). I stopped wearing my belt and the holes hold stopped appearing on new shirts.

Do the holes in your shirts take place in a similar place? If so, perhaps nearby is something wearing through the fabric. If not, it may be poor fabric competence.
